There are currently 6 forms ( a- f ) required for participation in the 2009 Camp del Corazon sessions. If you are interested in joining us, please submit the completed application forms by mail.

The Parent or Legal Guardian must complete and sign the:

	Camper Expectation Form (c)
	Camper Demographics Form (d)
	Camper Profile Form (d)

Your Pediatric Cardiologist must complete, sign, and return the:

Cardiology Medical Form (f)

   Authorization to Obtain Information
   Camp name request
