PACE Registration Guide

 

1. Make sure you are eligible for the PACE program.

(a) Any young adult between the ages of 18 and 25, or who is too old to attend camp is eligible to attend PACE.

(b) Must also have been diagnosed with congenital or acquired heart disease.

2. Apply to PACE program.

Every individual who is interested in participating in this program must apply and be accepted to the program prior to attending any PACE events.

(a) First download and fill out PACE application and send it in to office along with Reference form which requires you to provide two letters of recommendation.

(b) Once your application is received it will be reviewed by the program advisors and mentors. You will be notified of your status shortly after.

(c) After being accepted into the program you will then be eligible to attend all upcoming PACE events.

(d) Applications for the upcoming year will be accepted beginning in June, and all applicants will be notified of their status no later than November 1st.

3. Check out and register for PACE events.

Prior to attending, every PACE event will have an application and waver forms that will need to be filled out. Please take a look at the PACE events page for more information about these events. The registration forms are below.  Every individual who is interested in participating in this program must apply and be accepted to the program prior to attending any PACE events. Please fill in the above forms prior to sending the below forms in.

 


 

Pace Events Registration

There are currently 5 forms ( a- e ) required for participation in PACE events. A separate set of (5) forms is required for each different PACE event.

 

The Applicant must complete and sign the:

PACE Applicant Info Form (a)

PACE Event Application Form (b)

PACE Medication Release Form (c)

PACE Waiver Form (d)

 

Your Pediatric Cardiologist must complete, sign, and return the:

PACE Cardiology Medical Form (e)

Once your completed forms are received, more information will be sent to you. If you are unable to download and or print the forms, please contact us at (818)754-0312 to receive a packet by mail. Participants will be responsible for taking all medications during all PACE events. In addition to the camp advisors there will also be at least one RN onsite to assist with medical concerns. Please make a note on the Medical and Cardiology form if you would like to talk with a nurse to discuss activity participation and/or medical regimens prior to camp.

Participation in PACE events are Free to all leaders, but we do ask for a $25.00 processing fee to help cover mailing, printing and other costs. *Please send your payment with the PACE event application * this fee can be waived for financial hardship. Non-payment of this fee will NOT affect a leaders acceptance. Please make checks payable to Camp del Corazon. We work very hard to keep this camp at no cost to campers, but if you, your company, employer, or friends would like to make a tax deductible donation to Camp del Corazon, please go to our donor page. And remember, each family is responsible for getting their camper to the boat dock and signed in. Again, thank you for your interest and involvement in Camp del Corazon.
