Summer Staff Volunteer Forms

Before Filling out any of these forms, please make sure that you meet the requirements to become a Summer Counselor Volunteer. These are listed on the General Info Section on Becoming a Summer Staff Volunteer. Please Also check out the Summer Volunteer Job Descriptions.

If you are interested in joining us and can make the necessary commitments, please submit, by mail, all of the completed application forms.
